**EAST COAST FOOTBALL ASSOCIATION (ECFA) PARTICIPANT LIABILITY WAIVER**

 

 

**ACKNOWLEDGMENT OF RISK**

 

I,____________, hereby acknowledge that participation in football and related activities involves inherent risks of physical injury, including but not limited to injuries resulting from physical contact with other participants, falls, and collisions. I recognize that the nature of the sport can be physically demanding and poses a risk of harm.

 

**WAIVER OF LIABILITY**

 

In consideration of being allowed to participate in the East Coast Football Association (ECFA) activities, I hereby agree to release, waive, discharge, and covenant not to sue the ECFA, its officers, agents, or employees (hereafter referred to as "Releasees") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of the property belonging to me, while participating in such activity, or while in, on or upon the premises where the activity is being conducted.

 

**INDEMNIFICATION**

 

I also agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs they may incur due to my participation in ECFA activities, whether caused by the negligence of the Releasees or otherwise.

 

**MEDICAL PERMISSION**

 

I hereby give my consent to ECFA,its representatives, and my Team Ownership to contact medical emergency personnnel that may be deemed advisable in the event of injury, accident, and/or illness during my participation in ECFA activities.

 

**ACKNOWLEDGMENT OF UNDERSTANDING**

 

I have read this waiver of liability and indemnification agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am checking the box and completing this registration the agreement freely and voluntarily, and intend by my signature/checking the box in aknowledgement to be a complete and unconditional release of all liability (and can not be modified orally) to the greatest extent allowed by law.
